The Patient Intake Associate supports the patient intake process by ensuring all inbound referrals are quickly contacted, accurately documented, and properly progressed through initial screening and qualification steps. This role is focused on responsiveness, accuracy, and maintaining a clean intake pipeline to support timely service initiation.


What You Will Do:

Referral Follow-Up & Outreach 

  • Conduct timely outreach to inbound patient referrals from MCOs, hospitals, and community sources  
  • Ensure same-day or rapid follow-up on all assigned referrals  
  • Maintain consistent communication with patients and referral sources as needed  

Initial Intake & Information Gathering 

  • Collect basic patient information including demographics, location, and service needs  
  • Support verification of eligibility and payer requirements as directed  
  • Document intake details accurately in system of record  

Authorization Intake Support 

  • Assist in gathering required information for authorization initiation  
  • Ensure all intake documentation is complete and ready for submission  
  • Flag missing or incomplete information for escalation  

Data Entry & Pipeline Maintenance 

  • Maintain accurate and up-to-date records in intake systems (e.g., HealthCloud or CRM)  
  • Ensure all required fields and documentation are completed  
  • Support pipeline cleanliness and accuracy  

Disposition & Escalation Support 

  • Support dispositioning of referrals based on defined intake criteria  
  • Escalate complex eligibility or service questions to Intake Leads or Manager  
  • Ensure timely movement or closure of referrals  

Handoff Preparation 

  • Prepare “qualified” patient referrals for handoff to Intake Leads or downstream teams  
  • Ensure all required intake information is complete and organized  
  • Support smooth transition into qualification and authorization workflows Executes additional projects and responsibilities as assigned by supervisor, with a clear expectation of meeting defined timelines and delivering results within established deadlines

Qualifications:

  • 0–2+ years of experience in healthcare support, customer service, or administrative roles
