Job description

Functions as a member of the Ryan White Support Services (RWSS) team to ensure self-referred, externally referred, and internally referred clients are connected to appropriate services in a timely manner. Responsible for managing and maintaining the referral tracking process for all referrals received by the program.

The Intake Coordinator conducts pre-intake screening of potential clients living with HIV/AIDS to assess needs, determine program eligibility, and triage referrals to the appropriate AFPH program or staff member. For individuals eligible for Ryan White Supportive Services (RWSS), the Intake Coordinator will complete intake documentation, including Consent for Services, HIXNY Consent, Releases of Information, Client Health Indicator (CHI) documentation, Social Determinants of Health (SDOH) assessment, and entry into the AWARDS electronic health record.

For clients referred to other AFPH programs, the Intake Coordinator will route referrals to the appropriate Program Manager and follow up to ensure outreach and intake scheduling occurs. The Intake Coordinator is responsible for maintaining ongoing communication regarding referral status and updating the referral tracking log through intake completion or case closure.
