Community Healthcare of Texas has provided hospice and palliative care services since 1996.  Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them.

Community Healthcare of Texas is currently recruiting for a Hospice Intake Coordinator.



POSITION SUMMARY
Provide patient admissions through coordination of referrals and admission team nurses. 

ESSENTIAL FUNCTIONS
  • Receive and verify validity of patient referrals.
  • Coordinate with admission team pertinent information in a timely manner.
  • Verify patient insurance. 
  • Provide admission information to doctor's office, business office, admission nurse.
  • Various routine clerical duties and other assignments as needed. 
POSITION QUALIFICATIONS
  • Licensed Vocational Nurse (strongly preferred)
  • Healthcare intake experience required
  • Knowledge of insurance requirements to hospice care
  • 1+ years of hospice experience, preferred
