About Us

Home Health Advantage, Inc. is a Medicare-certified home health agency providing skilled nursing, physical therapy, occupational therapy, speech therapy, medical social services, and home health aide services throughout the Chicagoland area. We are committed to delivering exceptional patient care and outstanding customer service.

We are seeking an experienced, highly organized, and motivated professional to join our team as a Home Health Intake Team Lead Coordinator.

Position Summary

The Home Health Intake Team Lead Coordinator is responsible for the daily operations of the Intake Department, ensuring efficient processing of referrals, accurate verification of patient information and insurance benefits, timely coordination of admissions, and exceptional customer service.

Candidates must have prior home health intake experience. Applications without direct home health intake experience will not be considered.

Essential Duties and Responsibilities

Intake Operations

  • daily intake and referral management activities.
  • Review and process incoming referrals from hospitals, physician offices, case managers, skilled nursing facilities, and community referral sources.
  • Ensure timely processing of referrals and admissions.
  • Verify patient demographics, insurance coverage, eligibility, and authorization requirements.
  • Coordinate referrals with clinical supervisors and scheduling staff to facilitate timely admissions.
  • Monitor intake workflow and ensure all referrals are appropriately tracked and documented.
  • Identify and resolve intake barriers and service issues promptly.

Customer Service & Referral Relations

  • Serve as a primary liaison between referral sources, patients, families, and agency personnel.
  • Maintain professional relationships with hospitals, physicians, discharge planners, and case managers.
  • Ensure exceptional responsiveness and customer satisfaction.
  • Address concerns and resolve issues professionally and efficiently.

Compliance & Documentation

  • Maintain accurate documentation within the Electronic Medical Record (EMR).
  • Monitor intake documentation for completeness and accuracy.
  • Ensure proper handling of authorizations, physician orders, and referral documentation.

Qualifications

Required

  • Minimum of two (2) years of direct Home Health Intake experience in a Medicare-certified home health agency.
  • Prior experience processing home health referrals and coordinating admissions.
  • Strong knowledge of Medicare, Medicare Advantage, Managed Care, and authorization requirements.
  • Experience working with electronic medical record systems.
  • Exceptional organizational, communication, and multitasking skills.
  • Strong problem-solving and critical-thinking abilities.
  • Ability to work effectively in a fast-paced environment.

Preferred

  • Experience with WellSky/Kinnser,  Axxess, or other home health EMR systems.
  • Knowledge of home health eligibility requirements, insurance verification, and payer authorization processes.
