Reliant at Home is seeking a Full-Time Remote Home Health Weekend Intake Coordinator!
The hours of this position will be 9AM - 6PM, Thursday - Monday!

Job Qualifications:

Education: High School Graduate
Experience: 2-3 years Home Health intake experience preferred, LVN candidates accepted
Skills: Required computer proficiency and excellent interpersonal, organizational, and phone skills. Must be able to move at a fast pace.

Functions:

  • Receives and processes new referrals in a timely manner.
  • Facilitates care coordination with sales reps, referral sources, physician offices, patients, and agency clinical leaders.
  • Effectively communicates and relays accurate care coordination details to internal and external constituents in the referral coordination process.
  • Takes ownership of recording accurate information on the referral tracker and in the EHR and ensuring all referrals move through the process quickly and responds and communicates in a professional manner.
  • Demonstrates commitment, competency, and ability to handle a lot of details proficiently.
  • Exhibits Reliant at Home's Bluebird Promise.
