Job description

Job Purpose:
- This position is responsible for supporting the intake, screening, and enrollment process for home-delivered meals and nutrition programs at LIFECARE ALLIANCE. The Intake Specialist plays a crucial role in ensuring that eligible individuals receive the services they need by efficiently managing the intake process and maintaining high standards of client engagement and communication.
Key Responsibilities:
- Intake Processing & Client Engagement: Conduct initial screenings and assessments for individuals seeking services, ensuring accurate collection and documentation of client information. Engage with clients in a compassionate and professional manner to understand their needs and provide relevant information about available programs.
- Eligibility Determination & Communication: Evaluate client eligibility for services based on established criteria and guidelines. Communicate eligibility decisions clearly and effectively to clients, providing guidance on next steps and addressing any inquiries or concerns.
- Systems, Compliance & Data Entry: Utilize client management systems, databases, and referral portals to accurately enter and maintain client data. Ensure compliance with organizational policies and procedures, as well as relevant regulations and standards.
- Coordination & Internal Collaboration: Collaborate with internal teams and stakeholders to facilitate seamless service delivery and address any issues or challenges that arise during the intake process. Participate in team meetings and contribute to continuous improvement efforts to enhance the efficiency and effectiveness of intake operations.

Required Education:
- High school diploma or equivalent
Preferred Education:
- Preferred completion of an associate’s degree in human services, Business Administration, or a related field
Required Experience:
- Experience using client management systems, databases, or referral portals
Preferred Experience:
- Preferred familiarity with Medicaid, Older Americans Act programs, or other publicly funded services
Required Skills and Abilities:
- Ability to communicate over the telephone and in person with a diverse public, including older adults and handicapped individuals
- Knowledge of modern office practices and procedures
- Exceptional customer service skills
- Ability to answer multi-line telephone system
