The Foster Future Foundation is seeking a dedicated and empathetic Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote capacity. This full-time remote position allows you to work from anywhere while collaborating with a dynamic team dedicated to social work in a hospice center. Although primarily remote, there may be occasional evening and weekend hours to accommodate client needs or specific program activities. • Conduct thorough and comprehensive assessments to identify clients' social, emotional, and environmental challenges, ensuring a complete understanding of their circumstances.
• Design and implement personalized service plans that include specific goals, interventions, and progress tracking, adapting as necessary to meet client needs.
• Provide supportive counseling and assistance to individuals and families experiencing grief, loss, and other life challenges associated with hospice care.
• Work collaboratively with clients to connect them with community resources that will enhance their well-being and facilitate their coping processes.
• Deliver crisis intervention and risk assessment, developing safety plans as required to ensure clients' safety and emotional well-being.
• Engage with community agencies and organizations to coordinate services, advocate for clients, and enhance their access to vital resources.
• Participate actively in community outreach initiatives to educate the public about available services and support systems.
• Attend case conferences, team meetings, and relevant training sessions to foster professional development and ensure continuity of care.

As a Clinical Social Worker at the Foster Future Foundation, you will primarily serve individuals and families facing end-of-life situations. Many clients will be dealing with emotional distress, grief, and complex family dynamics, requiring a compassionate approach tailored to their unique needs. You'll play a critical role in helping clients navigate their feelings and support them in accessing necessary resources to enhance their quality of life during challenging times.

• A Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of social and economic justice issues and trauma-informed care principles.
• Outstanding communication skills, cultural competence, and an ability to approach problem-solving with creativity and sensitivity.

• Experience in a social service agency or similar setting, specifically within hospice or palliative care environments, is highly desirable.
• Familiarity with community resources and best practices for client advocacy in social work.
• Experience in delivering crisis intervention and working with diverse populations, including those struggling with grief.

The Foster Future Foundation offers a competitive salary range of $60,000 to $100,000 annually, commensurate with experience, alongside performance-based bonuses. A sign-on bonus is also available to new hires. We believe in taking care of our team, which is why we provide:
