In this remote role, you will collaborate with a dynamic and diverse team of professionals who share a passion for social justice and community outreach. • Conduct detailed assessments to identify clients' social, emotional, and environmental challenges, utilizing effective assessment tools and techniques.
• Design, implement, and monitor personalized service plans that include achievable goals, specific interventions, and regular progress check-ins to measure outcomes.
• Provide one-on-one counseling and support to individuals and families facing life's various adversities, fostering resilience and empowerment.
• Actively collaborate with clients to create individualized service plans, connecting them with essential community resources and support networks.
• Offer crisis intervention strategies, including risk assessments and safety planning, to help clients in high-stress situations stabilize their environments.
• Engage with external community agencies and organizations to coordinate services, fostering a strong advocacy network that supports client needs.
• Participate in community outreach initiatives to raise awareness of available agency services and resources.
• Attend case conferences, team meetings, and ongoing staff training sessions to enhance skills and knowledge while contributing to team goals.

Our primary focus is on vulnerable populations within the hospice care community, including individuals and families facing terminal illnesses. Your work will involve providing tailored support that acknowledges the emotional and psychological complexities of end-of-life care. You'll assist clients and their families in navigating the myriad challenges that occur during this critical time, ensuring they have access to essential resources and emotional support.

• A Master’s degree in Social Work from an accredited institution is required.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Strong foundation in social and economic justice, with a deep understanding of community resources and trauma-informed care practices.
• Exemplary communication, cultural competence, and problem-solving skills to effectively engage with diverse client populations.

• Experience working in a social service agency, hospice care, or similar settings is highly desirable.
• Deep knowledge of mental health issues, crisis intervention methodologies, and family dynamics in the context of hospice services.
• Familiarity with evidence-based practices that support client empowerment and community integration.

The salary for this position ranges from USD 60,000 to 100,000 per year, complemented by an attractive sign-on bonus.
