Work Environment

Key Responsibilities

• Conduct thorough assessments of clients to identify their social, emotional, and environmental challenges. For example, you will work with clients to understand their immediate needs, resource gaps, and any barriers to achieving their goals.
• Develop individualized service plans outlining specific goals, intervention strategies, and progress metrics, ensuring that these plans regularly adapt to meet your clients' evolving needs.
• Offer empathetic counseling and support to clients facing various challenges, employing active listening techniques and therapeutic modalities tailored to their unique situations.
• Guide clients through the development of personalized service plans while connecting them with and leveraging community resources, ranging from housing assistance to mental health services.
• Provide immediate crisis intervention and risk assessments, collaborating with clients on safety planning when necessary, particularly in situations involving domestic violence or suicidal ideation.
• Work closely with various community organizations to coordinate and advocate for necessary services on behalf of your clients, serving as a bridge between them and the resources they require.
• Engage in community outreach initiatives to raise awareness about the services we provide and the challenges faced by the populations we serve, enhancing agency visibility and establishing valuable community connections.
• Participate in regular case conferences, team meetings, and ongoing professional development opportunities to stay current with best practices and foster effective collaboration within the agency.

Patient Population / Client Focus

In this role, you will primarily support individuals and families from low-income backgrounds, many of whom face significant social and economic challenges. The client population may include those experiencing housing instability, mental health issues, substance use disorders, and trauma. Your ability to relate to and understand their unique struggles will be vital in providing effective support and achieving meaningful outcomes.

Qualifications

• A Master’s degree in Social Work from an accredited institution is required.
• An active state license as a Licensed Clinical Social Worker (LCSW) is mandatory to practice.
• Strong understanding of social and economic justice issues, community resources, and trauma-informed care principles is preferred.
• Exceptional communication, cultural competence, and problem-solving skills are critical for effective client interaction and advocacy.

Preferred Experience

• Prior experience working in a social service agency or a similar environment is preferred.
• Demonstrated success in case management, crisis intervention, or related roles enhances your fit for this position.

Compensation and Benefits
