The Community Liaison position is open in our Murrieta location. This is a Full-time position during our day shift from 8:00am-5:00pm.
Requirements of the Community Liaison:
โ€ข Licensed or unlicensed health professional with experience in business development
โ€ข Knowledgeable about home health care, hospice, managed care, community relations, home care trends and reimbursement issues
Community Liaison Duties Include:
โ€ข Spends 75% of time in the field selling/marketing VNAcare services, generating referrals for these services, and implementing strategies to increase sales of these services and referrals for these services.
โ€ข Provides professional and community education on agency programs and services.
โ€ข Participates in Community Health Fairs and events in respective territory.
โ€ข Participates in appropriate associations/service groups.
โ€ข Communicates effectively keeping the Director of Healthcare Partnerships apprised of activities in the territory. Acts as a liaison between the customer and the agency.
โ€ข Identifies new referral sources, develops and implements plans to build business and generate referrals.
โ€ข During field visits, monitors satisfaction of patients, referral sources, and other customers.
โ€ข Maintains Trella CRM to ensure appropriate and complete documentation. Submits weekly notes and schedule to Director of Healthcare Partnerships.
โ€ข Identifies and obtains contracts with appropriate facilities (RCFE's and SNF). Develops a relationship with these facilities to generate referrals.
โ€ข Provides education to physicians and office staff on Care Plan coordination.
โ€ข Assist in coordinating and obtaining timely physician review and signatures on care plans and supplemental orders within the assigned territory, in accordance with regulatory and organizational requirements.
