Job Title- Community Liaison – Home Health
Job description:
We are seeking a dynamic and results-driven Community Liaison for Home Health to serve as a vital bridge between healthcare providers, patients, families, and community organizations. In this role, you will actively promote home health services, build strong referral relationships, and expand access to care within the community.
This is a relationship-focused position requiring an individual who is already well-connected within the healthcare community and capable of generating and maintaining referral sources.
Responsibilities
  • Develop and implement outreach strategies to increase awareness and referrals for home health services through community engagement and provider relationships
  • Build, maintain, and grow relationships with physicians, discharge planners, case managers, social workers, and community organizations
  • Serve as a representative of the agency at meetings, health fairs, and professional events
  • Identify and develop new referral sources while maintaining existing accounts
  • Coordinate with internal teams to ensure smooth intake and patient transition into services
  • Support program development efforts aligned with community needs
  • Track outreach activities and referral trends to support growth strategies
  • Manage social media and marketing initiatives to increase visibility
