We are seeking a motivated, outgoing and results - driven Community Liaison to help grow our skilled homecare services through healthcare marketing, referral development, community outreach and relationship building

This is an excellent opportunity for a healthcare marketer, sales professional, community relations specialist, or business development professional who enjoys networking, building meaningful partnerships, and generating new referral opportunities. 

The ideal candidate is confident meeting with healthcare professionals and community leaders, comfortable presenting services, and motivated by achieving measurable growth while making a positive difference in the lives of patients and their families. 

Key Responsibilities

  • Develop and maintain strong referral relationships with hospitals, physicians, rehabilitation centers/SNFs, assisted living communities, senior centers, social workers, case managers and other healthcare professionals. 
  • Develop and implement marketing and business - development strategies to increase referrals and expand the agency's presence in the community. 
  • Identify and cultivate new referral sources and business opportunities
  • Conduct regular in person visits with referral partners and prospective partners. 
  • Represent the agency at networking events, community evens, health fairs, educational programs, and professional meetings.
  • Promote the agency's skilled homecare services through presentations, outreach and relationship - based marketing. 
  • Track referral activity, outreach efforts, and business - development results. 
  • Build long - term relationships with referral partners through consistent communication and follow up. 
  • Collaborate with the clinical and administrative teams to support a smooth admission process and exceptional customer experience. 
  • Monitor market trends and identify opportunities to expand the agency's reach and services
  • Represents the agency professionally while upholding patient privacy, ethical standards, and applicable regulatory requirements.

What We're Looking For

  • Marketing, sales, business development, healthcare marketing, hospice, home healthcare, or community outreach experience preferred
  • Proven ability to build relationships and generate new business/referral opportunities
  • Excellent verbal, written, presentation, and interpersonal communication skills
  • Confident, professional and comfortable with cold outreach, networking, and face - to - face meetings
  • Self - motivated and goal - oriented with the ability to work independently. 
  • Strong organizational and follow - up skills
  • Ability to manage multiple referral relationships and marketing activities
  • Basic computer proficiency and ability to maintain accurate activity records
  • Valid driver's license
