Hospice Community Liaison

LilyCare Hospice is a community-based hospice serving patients and families across the greater Albuquerque area. We provide care wherever patients call home — private residences, assisted living communities, and skilled nursing facilities. Our growth runs on trusted relationships with the referral community, built one conversation at a time. We are hiring a Hospice Community Liaison to grow our referral network and bring our care to more families across the metro.

Duties and Responsibilities

  • Own the greater Albuquerque territory as our Hospice Community Liaison — build and deepen referral relationships with hospitals, skilled nursing facilities, assisted living communities, and physician practices through consistent, well-planned sales calls

  • Execute a weekly, monthly, and quarterly territory strategy — prospecting, account development, and call routing — to grow market share and meet admission goals

  • Plan and deliver in-services and community education events that show medical and community groups what hospice offers and when to refer

  • Manage your marketing budget like it's your own money, projecting return on what you spend

  • Document activity in the CRM, report account progress at staff meetings, and partner with our clinical team on clean referral handoffs

Compensation and Schedule

The Hospice Community Liaison role pays $82,000–$87,000 per year plus performance bonus. Full-time, Monday–Friday, with occasional evenings and weekends for community events and referral-source availability. Territory is the greater Albuquerque area, based out of our office at 4811 Hardware Dr NE, Albuquerque, NM.

Skills and Qualifications

  • 2+ years of hospice experience (preferred)

  • Valid driver's license, current auto insurance, and reliable transportation for daily travel throughout the greater Albuquerque area (required)

  • Established Albuquerque referral relationships and prior work as a hospice community liaison or hospice marketer strongly preferred

  • Persuasive, outgoing communicator
