Hospice Liaison – Houston Territory

Join Our Team!
We are a leading national Hospice Care Organization seeking a compassionate and driven Hospice Liaison to cover an assigned territory in the Houston area. This role is critical in building strong relationships with referral sources and ensuring seamless coordination for patients entering hospice care.

As a Hospice Liaison, you will:

  • Develop new referral sources while maintaining and strengthening existing relationships.
  • Manage the completion of hospice referrals and ensure accurate, up-to-date patient records throughout the process.
