• Act as a vital liaison between patients, families, and outpatient providers to facilitate open communication and effective care coordination.
• Utilize your clinical skills to guide interactions with insurance companies for pre-certification and utilization management, ensuring patient access to essential services.
• Participate actively in discharge and aftercare planning, developing comprehensive post-care strategies to support clients in their transition back to home or community settings.
• Offer therapeutic meal support, demonstrating a healthy relationship with food and promoting nutritional well-being as part of emotional healing.
• Provide empathetic, affirming, and community-sensitive care tailored to the unique circumstances and intersecting identities of each client.

You will primarily work with older adults and their families who are navigating the complexities of end-of-life care. This includes assessing their psychological and emotional needs within the hospice care framework, and you will specifically tailor your services to honor their diverse backgrounds, cultural identities, and specific circumstances. Your expert guidance will empower clients and their families during this critical phase of their journey.

• A Master’s degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of California is mandatory for this role.
• Minimum of one year of experience working with older adults or in hospice care settings is essential.
• Strong clinical and diagnostic skills, with the ability to assess and address complex emotional and psychological issues.
• Excellent communication and interpersonal skills that facilitate effective teamwork and client interactions.
• A capacity to work both independently and collaboratively within a virtual team environment.

• Experience engaging with diverse populations in a hospice or mental healthcare setting.
• Proficiency in case management systems and electronic health records (EHR).
• Familiarity with therapeutic meal programs and nutritional counseling.

Integral Health Care Center offers a competitive salary ranging from USD 115,000 to 130,000 annually. In addition to a robust salary, our benefits package includes:
