You will play a significant role in supporting patients on their journey towards recovery and personal growth within a collaborative and patient-centered environment.

About the Organization

Integral Health Care Center is an established provider of comprehensive mental health services located in Southampton, New York. Our center specializes in a broad range of therapeutic modalities, addressing not only emotional health and wellness but also supporting individuals dealing with substance use recovery, developmental disabilities, and acquired brain injuries. We are deeply committed to promoting independence, community engagement, and overall wellness through person-driven programming. Our core values of respect, integrity, and compassion guide our interactions with clients, ensuring that every individual receives the highest standard of care tailored to their unique circumstances.

Work Environment

This full-time position is remote, allowing you the flexibility to work from your chosen environment while still being an integral part of the Integral Health Care Center’s team. Despite the distance, we foster a strong team culture that thrives on collaboration and communication, enabling you to build meaningful relationships with patients and colleagues alike. The outpatient office setting includes access to resources and support necessary for effective patient engagement and treatment delivery.

Key Responsibilities
  • Conduct comprehensive assessments and develop individualized treatment plans that include crisis management strategies, behavioral guidelines, and risk assessments as part of your approach to care.
  • Timely completion of all required documentation to ensure compliance with regulatory standards, maintaining the highest levels of accountability and professionalism.
  • Implement evidence-based screening tools in collaboration with the clinical team to optimize patient care and measure outcomes effectively.
  • Work closely with interdisciplinary teams to create and enforce positive behavior support plans, ensuring that all care strategies are tailored to the individual’s strengths and needs.
  • Actively participate in licensing and accreditation processes, contributing to the overall quality assurance of the services we provide.
  • Serve as a liaison with various service systems, including state agencies and community partners, to enhance care coordination and access to necessary resources.
  • Establish and maintain effective communication channels that align with contractual standards while furthering the mission and values of Integral Health Care Center within the community.
  • Engage with local, state, and federal agencies to promote programs, forge partnerships, and identify new funding opportunities that will benefit our patient population.
  • Continually pursue ongoing professional development through attending conferences, participating in workshops, and representing the organization in networking events.
  • Advocate for and integrate the agency’s mission, values, and principles into all aspects of your work, positively influencing the overall direction of the organization.
  • Take on additional duties and responsibilities as assigned by the Clinic Director to support organizational needs and patient care goals.
Patient Population / Client Focus

In this role, you will serve a diverse patient population, including individuals with mental health disorders, those in recovery from substance use, and clients with developmental disabilities. Your work will directly influence their mental well-being, helping them to achieve independence and recovery through tailored therapeutic interventions. You will have the unique opportunity to forge meaningful connections with each client, providing meaningful support in their personal journeys.

Qualifications
  • A Master’s Degree in a relevant mental health field is a fundamental requirement.
  • An independent LMHC license is also required, demonstrating your qualifications to practice independently.
Preferred Experience

While all qualified candidates are encouraged to apply, experience in outpatient mental health settings, familiarity with evidence-based practices, and background in working with diverse populations are advantageous. Ideal candidates will also possess strong communication skills and the ability to collaborate effectively in a remote work setting.
