We are seeking a Clinical Liaison Registered Nurse (RN) who excels in fast-paced healthcare environments and is passionate about enhancing the patient and family experience.


Position Summary

The Clinical Liaison RN serves as the primary clinical contact for hospice referrals, providing bedside assessments, admission coordination, and care transition support. This role partners closely with hospital teams, physicians, case managers, and internal hospice staff to ensure timely, seamless admissions and exceptional patient care.


Key Responsibilities
  • Respond to hospice referrals and perform real-time bedside clinical assessments
  • Evaluate patient eligibility for hospice services and support physician certification (CTI)
  • Educate patients and families about hospice benefits, services, and care options
  • Coordinate and facilitate hospice admissions, including documentation and consents
  • Collaborate with Transitional Care Navigators (TCNs), hospital staff, and physicians
  • Support transitions to General Inpatient (GIP) level of care when appropriate
  • Serve as an on-site clinical resource for urgent admissions and field staff support
  • Build strong relationships with referral partners and care teams
  • Ensure accurate, timely completion of admission documentation
  • Provide education to hospital staff on hospice eligibility and referral processes
  • Participate in after-hours and weekend coverage as needed

Required Qualifications
  • Graduate of an accredited Registered Nursing (RN) program
  • Active RN license in the state of practice
  • Current BLS and First Aid certification
  • Minimum 1 year of clinical nursing experience (hospice, palliative care, or acute care preferred)
  • Strong clinical assessment and decision-making skills
  • Excellent communication and interpersonal skills
  • Ability to travel 50%+ within assigned territory
  • Valid driverโ€™s license and auto insurance

Preferred Qualifications
  • Prior experience in hospice, palliative care, or care transitions
  • Knowledge of Medicare hospice eligibility requirements
  • Experience working in hospital or acute care settings
