Communicates with Admissions Director and team updates on potential residents throughout day and at end of day to prepare for the next mornings stand-up meeting.Establishes strong, effective external relationships with area hospitals and other community-based entities to promote referral development, admissions and potential "partnership" programs and services.Aggressively seeks out potential clients and referral sources with daily hospital rounds and with other referral resources, as needed. Works closely with Director of Business Development to build Assisted Living and LTC census.Follows hospitalized BGRV residents from BGRNC, AL, and Independent Living to determine discharge plan and needs, and ensures the provision of timely clinical information to appropriate BGRV facility prior to readmission.Performs on-site, home or other facility visits for pre-admission acuity assessments and determines if potential client meets the criteria for admission, reviewing assessment as needed with DON. Ensures the provision of timely clinical information and/or level of care to appropriate BGRV facility prior to admission.Responsible for case management rate determination with outside payers depending on the potential admission's medical needs; documents on appropriate forms.Meet potential residents and/or family members in hospital or at other facilities, to establish rapport, educate on Medicare services, schedule facility tours (when needed), and discuss facility services and features.Coordinates tour, completion of admission paperwork and admission time with Admissions Counselor.Conducts and participates in company open-house events as scheduled. Assists Marketing Department with trade shows, health fairs, and seminars as needed.Attends and participates in staff meetings, seminars, and trainings as scheduled.Participates in admissions team on-call rotation as scheduled.Interact with residents, visitors, staff, and general public in a professional and helpful manner while performing daily duties.Maintains daily inquiry, admission and referral source information on appropriate forms.Has the ability to work assigned work hours.Requirements of a Community Nurse Liaison: Must have excellent customer service skills and competencies, and the proven ability to develop effective referral relationships with potential and existing customers. Ability to interact effectively with employees, colleagues, customers, clients and vendors at all levels.Must be able to work with and assess all patient types. Must be able to work alone and set own goals which match those of the facilities and hospitals.
