RNCM to join our S&S Team to cover
Newburyport, Mass Territory
(Will include coverage for NH & Maine peripheral support if needed)
Constellation Health Services is seeking a Registered Nurse to join our start-up and survival team (S&S). As we expand our footprint, presentation, articulation, and a strong skill set are crucial for introducing our brand. Team members will manage the initial setup and transition to the permanent clinical team through onboarding and training. They will be or will become MatrixCare power users. Our team ensures seamless operations during growth, extended PTO, or other gaps. These team members prevent extra strain on the team, keeping patient care our top priority.

Responsibilities:
  • Delivery of all aspects within their scope of practice, RN CM, On call, Aides services where needed, and role has not been fulfilled yet by a home health aide in all environments that a patient calls home (skilled nursing facility, home, etc)
  • Deliver compassionate and comprehensive nursing care to patients facing end-of-life challenges.
  • Conduct thorough assessments to identify patient needs and develop individualized care plans.
  • Manage medications and treatments as prescribed by the physician.
  • Provide emotional support and guidance to patients and their families.
  • Educate patients and families about hospice care and end-of-life issues.
  • Collaborate effectively with interdisciplinary team members to ensure coordinated care.
  • Maintain accurate and up-to-date patient records.
  • Ability to travel up to 100% of the time within the assigned geography.

Qualifications:
  • Current and active RN license in the state of Ohio
  • Bachelor's degree in nursing (BSN) preferred.
  • Minimum of 2 years of experience in hospice
  • In-depth understanding of end-of-life care principles and practices and certified home health.
  • Exceptional communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Compassionate and empathetic approach to patient care.
  • Matrix and Enclara experience helpful
  • Basic knowledge of iOs operating system.
