Job description

We are seeking an experienced and compassionate RN Clinical Manager with a strong background in home health to join our leadership team. This individual will play a pivotal role in overseeing clinical operations, ensuring the highest quality of patient care and maintaining regulatory compliance within our home health agency. The RN Clinical Manager will serve as a mentor and leader to the clinical staff, driving performance, patient outcomes, and operational excellence.
The Clinical Manager coordinates and oversees all direct care patient services provided by clinical personnel:
  • Develops, plans, implements, analyzes, and organizes clinical operations.
  • Supervise and support clinical staff including RNs, LPNs, therapists, aides, and other home health professionals.
  • Review and approve patient assessments, care plans, and documentation.
  • Conduct routine case conferences and participate in interdisciplinary team meetings.
  • Monitor clinical performance, caseloads, and patient outcomes.
  • Provide field supervision and performance evaluations as needed.
  • Collaborate with the intake, scheduling, and administrative teams to ensure efficient operations.
  • Assist in hiring, onboarding, and training new clinical staff.
  • Participate in quality improvement initiatives and ensure documentation accuracy.
  • Serve as a liaison between clinical staff, physicians, patients, and families.
Required Experience/Skills
  • Graduate of an accredited School of Nursing.
  • Registered Nurse (RN) license in good standing, required.
  • Clinical leadership experience in a Medicare-certified home health agency.
  • Strong understanding of PDGM, QAPI and care coordination.
  • Passion for delivering high-quality patient-centered care in a home setting.
  • Must know OASIS and two years as a Registered Nurse with at least two year of management experience in home care.
