Job description

Reporting to the Clinical Care Manager, the Clinical Care Manager Assistant (CCMA) is responsible for assisting the CCM and the team nurses in ensuring the smooth functioning of the teams. The CCMA facilitates the timely completion of paper work and data entry relating to patient care documentation. The CCMA demonstrates knowledge of the EMR computer program and of proper documentation procedures. The CCMA demonstrates professionalism in speaking with patients and their families/caregivers, and with healthcare professionals and outside vendors. The CCMA takes responsibility for updating his/her knowledge of systems and procedures as these change. The CCMA supports the organizational vision and mission.
EDUCATION AND QUALIFICATIONS:
  • Currently licensed in the State of Florida as a Licensed Practical Nurse (LPN), with a minimum of one (1) year of nursing experience

ESSENTIAL JOB RESPONSIBILITIES:
Data Entry
  • Enters all patient status changes into the appropriate EMR screen in a timely manner.
  • Keeps team census spreadsheet up-to-date.
  • Maintains accurate disaster preparedness spreadsheet.
  • Maintains patient oxygen vender usage spreadsheet.
  • Prepares daily status reports.
  • Enters care plan templates for interdisciplinary group meetings (IDG).

INTERDISCIPLINARY GROUP MEETINGS
  • Prepares and distributes agenda for weekly IDG.
  • Runs recertification alert report weekly to ensure that patients' recertification are done in a timely manner.
  • Ensures that recertification paperwork is correct and complete, and brings it to IDG meeting.
  • Ensures that all Medical Director Certifications for new admissions are complete.
  • Prepares sign-in-sheet for IDG meetings and makes sure that it is signed by all attendees.
  • Sets up IDG computers for self and CCM in meeting room and runs EMR on screen for team to follow.

NOTIFICATION
  • Notifies all team members of patient status changes.
  • Notifies O2 Solutions of patient status changes.
  • Notifies outside doctors of patient deaths.
  • Sends updates and copies of new orders to JSA physicians and to any other outside physician that requests same.
  • Sends updates on VA patients to VA Palliative Care Team.

MISCELLANEOUS
  • Check patient folder for completion after patient discharge or death, and give to CCM for signature, and then to HIM.
  • Print patient care plans weekly after each IDG and ready them for team signatures. When signed by all team members, deliver to HIM.
  • Run "unverified visits" report weekly and notify team nurses if they have unverified visits.
  • Distribute weekly audit report to nurses that have deficiencies.
  • Print out nursing schedule sheets each week for team nurses to fill in.
  • Run productivity reports upon request

SPECIAL TASKS: Each CCMA has special tasks that only he/she is responsible for. These may include:
  • Running GHS census reports.
  • Verifying Enclara monthly billing against EMR patient status information.
  • Ordering office supplies.
  • Faxing answering service reports to team members.
  • Copying and distributing weekend and overnight triage reports.

CUSTOMER SERVICE
  • Maintains a professional relationship and serves as a liaison between all team disciplines.
  • Offers emotional support to the patient/family in accordance with the hospice philosophy and plan of care.
  • Observes Patients' Rights.
  • Maintains a positive attitude and caring spirit. Displays a "can do" attitude.
  • Comes to work as scheduled and consistently demonstrates dependability and punctuality.
  • Demonstrates honesty and integrity in all matters.

SKILLS AND COMPETENCIES:
  • Maintains up to date skill sets as required to care for patients
  • Possesses ability to multi-task; exhibits adaptability and flexibility in meeting Gulfside Healthcare Services objectives
  • Ensures compliance with all patient care and documentation standards and regulations
  • Assists in the implementation and maintenance of the policies and procedures of the clinical department; adheres to and assists in the evaluation of the same
  • Assists in maintaining federal and state regulatory compliance
  • Proficient in Electronic Medical Records
