Calling all Passionate Registered Nurses (RN) to join our fast paced Liaison team!
This position will be working at Winthrop Hospital in Mineola

Good Shepherd helps patients and their families live each day fully, focusing on quality of life, control of symptoms and the things that matter to them the most.
Job Details
Our Hospice Liaison RN's :
  • Reviewing patient referrals, discuss hospice care options with patients and family members, and plan with both the patient and the facility to prepare for hospice care.
  • Develops a positive working relationship with doctors, nursing facilities, and hospitals who refer clients to hospice.
  • Coordinate discharges
  • Execute and follow up on referrals
  • Advocate for the patient and family and why they should choose Catholic Health
  • Act as a liaison to referral sources on behalf of Catholic Home Care and Good Shepherd
  • Identify potential opportunities for market growth and Hospice program expansion

Qualifications:
  • Active New York State RN or MSW License
  • Excellent interpersonal, marketing and customer service skills
  • Two years Hospice/Home Care experience preferred
  • Strong computer and EMR experience.

Posted Salary Range
USD $100,000.00 - USD $115,000.00 /Yr.
