Job description

Hospice Liaison / Care Consultant — Company Car + Uncapped Commission

Competitive Base + Uncapped Commission | $200K+ Top Earners | Company Car Included

Full-Time | Field-Based

You shouldn't have to apologize to referral sources for the hospice you represent.

Hospice Care Consultant (HCC) role — company car included. Uncapped commission with top performers earning $200K+. Join one of the fastest-growing hospice providers in the Midwest, South, and Mid-Atlantic.

This role is for the experienced hospice sales rep, the healthcare account executive ready to grow, or the clinician who's realized they'd be great at this. What matters is that you care, you connect, and you follow through.

About Moments Hospice

Moments Are Forever. Changing the Hospice Experience, One Moment at a Time.

Moments Hospice provides compassionate end-of-life care across 29 locations in the Midwest, South, and Mid-Atlantic. We're one of the fastest-growing hospice providers in the country — and we've grown because of how we do this work, not just how much of it we do. We call it The Moments Way: present, responsive, and built around what each patient and family actually needs. It's a culture of yes — and when an HCC brings us a referral, our clinical and operations teams show up the way we promised they would.

Moments was built by people who wanted to do hospice differently. We celebrate wins together, pick each other up on hard days, and invest in the people who invest in us. Our values — Compassion, Integrity, Collaboration, Excellence, Celebration — aren't posters on a wall.

Who We're Looking For

You're a relationship builder. Healthcare or hospice sales experience is a great head start — but it's not the only path. Some of our top HCCs came in as seasoned sales reps, others came from clinical backgrounds, and some came in newer to sales but with the drive, empathy, and follow-through that make this work. What they share is the ability to build real relationships and represent something they believe in — and if that's you, you're closer to right for this role than you might think.

Backgrounds we've seen succeed: hospice or home health sales, medical device or pharma, clinical liaison, RN liaison, nursing/social work/chaplaincy ready to step into sales, SNF or AL business development, outside sales from other industries, or anyone ready to step into hospice sales with the right drive and heart.

Compensation & Benefits

 • Truly uncapped commission — top performers earning $200K+
 • Strong base salary with room to grow as your territory grows
 • Company fleet vehicle for business and personal use — fuel and insurance included
 • Expense account
 • Medical, dental, vision + life insurance
 • 401(k) with company match
 • Wellness benefits including telehealth, mental health support, and EAP
 • Professional development support, including continuing education
 • Generous PTO from day one
