Home Health Care Liaison

Join our team at Deer Meadows Home Health Services in Philadelphia, Pennsylvania

We are seeking a high-performing Home Health Care Liaison who knows how to generate referrals, build census, and produce measurable results.

If you already have relationships with discharge planners, social workers,

MLTCs, or community referral sources—and you know how to convert them

into admissions—we want to speak with you.

This role is not entry-level. Must have demonstrated success in Home Care

Marketing with ability to independently generate referrals through strong

professional presence and follow-through.

Home Health Care Liaison Key Responsibilities

  • Develop and manage referral relationships with hospitals, rehab

centers, nursing homes, clinics, physicians, social workers, and

community organizations

  • Drive consistent patient referrals and increase agency census

  • Represent the agency professionally at community events, meetings,

and outreach initiatives

  • Maintain ongoing communication with referral sources to ensure

satisfaction and repeat referrals

  • Track outreach activity and referral results

  • Work closely with intake and operations teams to ensure smooth

admissions

Home Health Care Liaison Requirements

  • Minimum 3 years of home care marketing experience

  • Established referral source relationships preferred

  • Strong knowledge of Home Health Medicaid, MLTCs, and private pay home care

  • Excellent communication and relationship-building skills
