RN Case Manager

Are you an energetic and compassionate RN ready for an immediate opportunity? We're seeking a dedicated Case Manager to coordinate vital care services for elderly and disabled individuals within the Home and Community Based Waiver Program. This role offers the chance to truly empower clients and ensure they receive the support they need to thrive at home.

Requirements to apply (one of these is essential):

  • RN or LPN license.
  • Bachelor's degree in Social Work, Human Services, or a related field.
  • Bachelor's degree in any field with 2 years of Human Services experience.
  • Associate's degree in behavioral science, social science, or a closely related field, plus 2 years of Human Services-related experience.
  • 3 years of demonstrated Human Services-related experience (examples include CMA, CNA, Case Management, Home Health, and Personal Care Assistant).

Beyond your qualifications, you'll need:

  • Strong communication and organizational abilities.
  • The capacity to manage multiple tasks effectively.
  • Flexibility for 50-75% travel.
  • The ability to pass all required background checks and drug screenings.

Prior experience with the Home and Community Based Waiver Program is a plus, but it's not a strict requirement. We'll discuss compensation during your interview.
