  • Oversee review of patients' medical records to ensure proper utilization of hospital services
  • Assist case managers in timely coordination of care and discharge planning
  • Screen all admissions to determine the appropriate level of care
  • Supervise case managers and oversee daily operations of the case management department
  • Facilitate optimal financial and clinical outcomes
  • Perform fiscal planning and ensure regulatory compliance
  • Analyze current systems and variances to identify opportunities for improvement
  • Promote quality of care through collaboration with the interdisciplinary team
Requirements
  • RN licensure in the state where the hospital resides required
  • Social workers may be accepted in lieu of RN licensure if licensed or holding a bachelor's degree in social work, as required and in accordance with state regulations
  • MSW, LSW, PT, OT, SLP and RT licensure may be accepted in accordance with state regulations
  • Current BLS certification required
  • Three to five years of case management experience required
  • Prior experience in IRF or LTACH preferred
  • Prior management experience preferred
  • BSN/MSN preferred
  • Certification in an approved Case Management Program preferred
