What You Will Do

Responsibilities include but are not limited to: review of assessment completed by Master of Social Worker (MSW) and supports the treatment plan, teaching and educating of participant, caregiver or other appropriate representatives under the supervision of the MSW. Social work interventions could include individual participant contacts; appropriate collateral contacts; participant and family education; provision of resources; ongoing case management; advocacy to ensure participant and caregiver needs are met and addressed; and disenrollment procedures under the supervision of the MSW. The Social Services Coordinator may be the assigned liaison between the caregiver representatives and community agencies.

Skills & Requirements

Bachelor’s degree in Social Work, Psychology or related field is preferred.

Two (2) years of experience working on a multidisciplinary team in a hospital, nursing home or community-based setting is preferred.
