Douglasville Center for Nursing and Healing is hiring a Social Services Assistant!!

Douglasville Center for Nursing and Healing, where rehabilitation and patient care is our top priority, is seeking a caring and passionate Social Services Assistant to join our social services team. As a Social Service Assistant, you will play a vital role in identifying individual social and emotional needs, developing and maintaining individualized care plans, and participating in interdisciplinary care plan meetings to ensure the best possible outcomes for our residents.

Responsibilities:

  • Identify individual social and emotional needs of residents

  • Develop and maintain individualized care plans

  • Participate in the planning of resident admission, community re-entry, or transfer to another facility

  • Participate in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of residents

Requirements:

  • Previous experience working in long-term care preferred

  • Bachelor's degree in Sociology preferred
