Position Overview:
As a Social Services Assistant, you provide vital support to social service department and residents. Your goal is to aid social services in offering community services that are most applicable and helpful to them. As a Social Service Assistant, you have the opportunity to change our resident's lives by providing aid many don't even realize is available to them. Your goal is to identify needs and work with the department and community leaders to see how we can meet the resident's needs.
Key Responsibilities:
  • Must keep accurate, updated, organized records after every resident visit to ensure that there are no undocumented breaks between treatments or visits.
  • Respond timely and answer questions about benefits that are available to residents and their family members or refer them to the correct organization to get help.
  • Review and submit reports regarding any problems with a resident.
  • Interview residents and/or family members when complaints or concerns are received. Must ensure full compliance of reporting any suspected abuse and/or violation of resident rights immediately to Social Services Director and Executive Director.
  • Must work closely with director and other department leaders to ensure resident's needs and complaints have been addressed.
  • Interview family members or individuals to obtain a complete family history to more closely determine which services are appropriate and needed.
  • Must demonstrate empathy, courteous, kind and professional workplace behavior and customer services to all residents, care team members, vendors, visitors and family members at all times.
  • Attends all mandatory in-services; and completes timely all necessary Relias trainings scheduled to perform. Keeps abreast and/or discusses with supervisor or manager all necessary policies, procedures and business practices within the scope of the position held to effectively perform all duties assigned.
  • Perform other tasks as assigned and within scope of practice.

Qualifications:
  • High school diploma or GED required.
  • Associate's degree preferred.
  • Social Services Designee, preferred, but not required.
  • Experience working in a long-term care setting or the aging population preferred.
  • CNA/SRNA/STNA preferred but not required.
