Job description

Our skilled nursing facility is seeking a Social Services Director to join our team in Brevard County- Melbourne, Viera or Rockledge area. The ideal candidate will be energetic, result-orientated team leader with superior interpersonal and communication skills to take charge of the Social Services Department.

Key Job Functions of the Social Services Director:

Manages department to assure assessments, discharges and psychological needs of residents are met.

Orient resident to long term care environment and facilitate adjust upon placement.

Timely and accurate completion and documentation of all required MDS for case load.

Understands the MDS process and accurately assesses, documents and provides interventions in the areas of hearing, speech, vision, cognitive patterns, mood, behavior and customary routines, discharge and communicates interventions to the care team.

Keeps in touch with and has professional relationships with vendors such as Hospice, Home Care, durable medical equipment (DME) companies.

Understand, comply and promote rules regarding all resident rights, promotes positive relationship with residents, visitors and regulators.

Responsible for the Medicaid process, completing MDS assessments, admission process etc.

Requirements of the Social Services Director:

Must have at least (1) one-year Social Services experience in a Skilled Nursing Facility

Bachelor’s degree in social work or another allied field, required.

Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing social services.
