We are currently searching for a Social Services Director
Job Summary:
• Responsible for the provision of Social Services to resident of the facility and their families under the direction of the social service director
• Must have working knowledge of interaction with residents of this facility. Job will include meeting new resident upon admission, completing an Initial Psychosocial Assessment, visiting residents to follow up on any concerns they may have. May complete some MDS assessments under the supervision of the social services professional.
• Arranging to purchase needed items for residents and ensuring that residents have adequate clothing items.
• Scheduling visits to the dental doctor, eye doctor, podiatrist and audiologist.
• Provide support in time of loss to residents and family
• Assist residents in gathering supporting documents needed for various support services as Medicaid, Medicare, SSI etc
• Any other duties that may be directed by the Social Services Director.
Must have the following:
  • Basic Knowledge of the operation of Long-term Care facilities: the effects of disease, loss, loneliness, boredom, on residents, their families and friends.
  • Knowledge of available community resources.
  • Ability to work within a team and respond positively to directions from the head of the Social Services Department.
  • Interest in a various age group who comprise the facility population. The ability to provide a positive influence on the lives of residents and their families.
  • Ability to communicate effectively, utilizing good verbal and writing skills.

Qualifications:
The social worker must meet the following qualifications:
  • Minimum of a Bachelor's degree in social work; OR
  • Minimum of a Bachelor's degree in a human-services related major, including sociology, gerontology, psychology, rehabilitation counseling or special education; AND
  • One year in a healthcare setting where the individual is working as a supervised social worker who works directly with individuals
