Job description

The Charlwell House is currently hiring for a part-time Social Worker who will be an assistant to the Director of Social Services. The assistant will be responsible for coordinating the social services of the long-term residents including: Care plan meetings, documentation, MDS documentation, and assistance with admissions and discharges, as well as other Social Service duties as needed.

This position requires 16 hours working on site.
