Licensed Master Social Worker (LMSW) – Hospital/Inpatient Contract Opportunity

Location: New York, NY
Pay Rate: $55–$60/hour (based on experience)
Employment Type: Full-Time Contract
Schedule: Multiple schedules available (day, evening, and weekend options)

Position Overview

We are seeking experienced Licensed Master Social Workers (LMSWs) for full-time contract opportunities within hospital and inpatient settings across New York City. This role is ideal for social workers with acute care or inpatient experience who thrive in fast-paced clinical environments and are passionate about supporting patients through care transitions, discharge planning, and psychosocial support.

Key Responsibilities

  • Conduct psychosocial assessments to evaluate patient needs, barriers to care, and support systems
  • Collaborate closely with interdisciplinary care teams to support treatment planning and continuity of care
  • Assist with discharge planning, including care coordination, placement support, and referrals to post-acute or community-based resources
  • Provide crisis intervention, emotional support, and patient/family counseling as appropriate
  • Coordinate with medical staff, case managers, and outside providers to facilitate safe care transitions
  • Maintain timely and accurate documentation within electronic medical record systems
  • Support patients navigating complex medical, social, and behavioral health needs during hospitalization
  • Help identify and connect patients to community resources, benefits, and support services following discharge

Qualifications

  • Active Licensed Master Social Worker (LMSW) license required
  • Master’s degree in Social Work (MSW) from an accredited program required
  • Minimum of 1 year of hospital or inpatient LMSW experience required
  • Strong understanding of discharge planning, care coordination, and psychosocial assessment in a clinical setting
  • Excellent communication, organizational, and interdisciplinary collaboration skills
