Position Description

LPNs play an essential role in our facilities and their skills are vital to the care we provide, making a difference in patients’ lives every day. Cadia Healthcare is dedicated to providing the best care possible and as a Licensed Practical Nurse, that’s exactly what you’ll do! By directing the duties of nursing assistants and performing day to day nursing duties, you’ll ensure our residents are fully cared for. We’re excited for you to come and join our accomplished staff of medical professionals!

Duties and Responsibilities

  • Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations & guidelines that govern the long-term care facility.  Meet with your assigned nursing assistants to give a Nursing Report on assigned residents and review 24-hour summary reports in the electronic medical record on assigned residents.
  • Perform administrative duties, such as completing medical forms, evaluations, observations, events & appropriate documentation for all assigned residents.
  • Ensure all medications & treatments have been signed off prior to the end of the assigned shift and update the Nursing Supervisor on resident changes in condition.
  • Receive telephone orders from physicians, transcribe orders & record in the electronic health record, and prepare and administer medications and treatments as ordered by the provider.
  • Inform nursing staff of new admissions, their expected time of arrival, room assignment, etc. and ensure that rooms are ready for new admissions, greet residents upon admission, and escort them to their rooms as necessary.
  • Make rounds with providers as necessary and make periodic checks to ensure resident care is being completed by CNA/GNA & evaluate resident status.
  • Ensure that your nurse’s notes reflect that the care plan is being followed when administering nursing care or treatment and review care plans for appropriate resident goals, problems, approaches, & revisions based on nursing needs.  Update care plans as appropriate.
  • Must possess a current, unencumbered, active license to practice as an LPN in this state or compact state.
  • Must possess a current CPR license.
  • Previous long-term care experience is desirable.
