Job description

Job Overview:
The individual in this position will work collaboratively with the behavioral health team to provide care coordination for patients in the inpatient behavioral health unit and patients who are consulted to behavioral health throughout DMMC.
Education:
Bachelor's in social work (BSW) and/or previous experience working in mental health preferred
Shift: Monday-Friday Days, rare holiday/evening/weekend hours as needed
Hours: 36 hrs/week
