The Patient Intake Associate supports the patient intake process by ensuring all inbound referrals are quickly contacted, accurately documented, and properly progressed through initial screening and qualification steps. This role is focused on responsiveness, accuracy, and maintaining a clean intake pipeline to support timely service initiation.

Responsibilities

What You Will Do:

Referral Follow-Up & Outreachย 

  • Conductย timelyย outreach to inbound patient referrals from MCOs, hospitals, and community sourcesย ย 
  • Ensure same-day or rapid follow-up on all assigned referralsย ย 
  • Maintain consistent communication with patients and referral sources as neededย ย 

Initial Intake & Information Gatheringย 

  • Collect basic patient information including demographics, location, and service needsย ย 
  • Support verification of eligibility and payer requirements as directedย ย 
  • Document intake details accurately in system of recordย ย 

Authorization Intake Supportย 

  • Assistย in gathering required information for authorization initiationย ย 
  • Ensure all intake documentation is complete and ready for submissionย ย 
  • Flag missing or incomplete information for escalationย ย 

Data Entry & Pipeline Maintenanceย 

  • Maintainย accurateย and up-to-date records in intake systems (e.g.,ย HealthCloudย or CRM)ย ย 
  • Ensure all required fields and documentation are completedย ย 
  • Support pipeline cleanliness and accuracyย ย 

Disposition & Escalation Supportย 

  • Support dispositioning of referrals based on defined intake criteriaย ย 
  • Escalate complex eligibility or service questions to Intake Leads or Managerย ย 
  • Ensureย timelyย movement or closure of referralsย ย 

Handoff Preparationย 

  • Prepare "qualified" patient referrals for handoff to Intake Leads or downstream teamsย ย 
  • Ensure all required intake information is complete and organizedย ย 
  • Support smooth transition into qualification and authorization workflowsย Executesย additionalย projects and responsibilities as assigned by supervisor, with a clear expectation of meeting defined timelines and delivering results within established deadlinesย

Qualifications

Qualifications:

  • 0-2+ years of experience in healthcare support, customer service, or administrative roles
  • Experience with CRM or intake systems (e.g., HealthCloud or similar) a plus
  • Strong communication and organizational skills
  • Healthcare, home care, or patient access experience preferred but not required
