Key Responsibilities
โข Process incoming medical records, faxes, scanned documents, and correspondence within established turnaround times
โข Upload documents accurately into the Electronic Medical Record (EMR) system while ensuring proper patient identification and document classification
โข Review records for completeness, accuracy, duplicate status, and correct dates of service before finalizing uploads
โข Coordinate and manage medical record requests from internal departments, patients, and external healthcare providers
โข Obtain and upload external records from hospitals, specialists, imaging centers, and other healthcare facilities
โข Track and follow up on outstanding record requests to ensure timely completion and resolution
โข Assist in obtaining prior medical records for new patient appointments
โข Maintain detailed and auditable documentation of requests, follow-up activities, and record status updates within the EMR
โข Monitor referral and imaging-related documentation to support continuity of care
โข Ensure strict compliance with HIPAA regulations, patient privacy requirements, and company policies
โข Meet departmental productivity, accuracy, and workflow standards
โข Identify opportunities for process improvement and workflow optimization within the medical records function
โข Perform additional duties as assigned
Qualifications
โข High school diploma or equivalent required
โข Previous experience in a medical records, healthcare, or clinical office environment preferred
โข Experience working with Electronic Medical Records (EMR) systems and document management processes
โข Knowledge of HIPAA regulations and medical record confidentiality requirements
โข Strong organizational skills and ability to manage multiple priorities effectively
โข Exceptional attention to detail and commitment to data accuracy
โข Strong written and verbal communication skills
โข Ability to work independently and follow established procedures consistently
โข Proficiency with Microsoft Office applications and computer-based systems
โข Knowledge of medical terminology preferred
โข Ability to maintain professionalism and confidentiality when handling sensitive patient information
โข Bilingual in English and Spanish or English and Creole preferred
