Key Responsibilities
• Process incoming medical records, faxes, scanned documents, and correspondence within established turnaround times
• Upload documents accurately into the Electronic Medical Record (EMR) system while ensuring proper patient identification and document classification
• Review records for completeness, accuracy, duplicate status, and correct dates of service before finalizing uploads
• Coordinate and manage medical record requests from internal departments, patients, and external healthcare providers
• Obtain and upload external records from hospitals, specialists, imaging centers, and other healthcare facilities
• Track and follow up on outstanding record requests to ensure timely completion and resolution
• Assist in obtaining prior medical records for new patient appointments
• Maintain detailed and auditable documentation of requests, follow-up activities, and record status updates within the EMR
• Monitor referral and imaging-related documentation to support continuity of care
• Ensure strict compliance with HIPAA regulations, patient privacy requirements, and company policies
• Meet departmental productivity, accuracy, and workflow standards
• Identify opportunities for process improvement and workflow optimization within the medical records function
• Perform additional duties as assigned
Qualifications
• High school diploma or equivalent required
• Previous experience in a medical records, healthcare, or clinical office environment preferred
• Experience working with Electronic Medical Records (EMR) systems and document management processes
• Knowledge of HIPAA regulations and medical record confidentiality requirements
• Strong organizational skills and ability to manage multiple priorities effectively
• Exceptional attention to detail and commitment to data accuracy
• Strong written and verbal communication skills
• Ability to work independently and follow established procedures consistently
• Proficiency with Microsoft Office applications and computer-based systems
• Knowledge of medical terminology preferred
• Ability to maintain professionalism and confidentiality when handling sensitive patient information
• Bilingual in English and Spanish or English and Creole preferred
