Job description

The Medical Records Clerk is responsible for maintaining, organizing, and safeguarding patient medical records in accordance with HIPAA guidelines and office policies. This role ensures the timely processing of medical record requests while maintaining accuracy and confidentiality.
Key Responsibilities

  • Receive, process, and track medical record requests from patients, providers, and third parties
  • Ensure all record releases are compliant with HIPAA regulations
  • Scan, file, upload, and organize patient documents in the electronic medical record (EMR) system
  • Verify patient information and ensure records are complete and accurate
  • Respond to phone calls and patient inquiries regarding medical records
  • Maintain confidentiality and security of all patient information
  • Assist with chart audits and document retrieval as needed
  • Perform other administrative duties as assigned
Qualifications
