Department/Specialty: Adult Inpatient

Schedule: Every Weekend, Saturday and Sunday 2x12 hours shift. To discuss start and end times

How you’ll make an impact in this role
  • Assess complex emotional and social barriers to develop personalized care plans that address each patient’s unique needs and preferences.
  • Coordinate with interdisciplinary teams to streamline the transition from admission to discharge, ensuring a safe and continuous journey of care.
  • Advocate for patient rights and preferences within the medical system to ensure the care provided is respectful and person-centered.
  • Empower patients and families through education on financial and community resources, helping them build the stability needed to manage their diagnosis.

What minimum requirements you’ll need

Licensure / Certification / Registration:

  • Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.
  • Certified Social Worker preferred.

Education:

  • Bachelor's degree in Social Work required.

Work Experience:

  • 3 years of experience preferred.

What additional preferences we're seeking
  • Must have Social Worker license.
  • Masters degree in Social Work required.
