Your future role at a glance

Location: Indianapolis, IN

Facility: Ascension St. Vincent

Department/Specialty: Emergency Department

Schedule: Wednesday-Saturday, 4x10 hours day shift, 11:00AM-9:30PM

How you'll make an impact in this role
  • Assess complex emotional and social barriers to develop personalized care plans that address each patient's unique needs and preferences.
  • Coordinate with interdisciplinary teams to streamline the transition from admission to discharge, ensuring a safe and continuous journey of care.
  • Advocate for patient rights and preferences within the medical system to ensure the care provided is respectful and person-centered.
  • Empower patients and families through education on financial and community resources, helping them build the stability needed to manage their diagnosis.
What minimum qualifications you'll need

Licensure / Certification / Registration:

  • One or more of the following required:
    • Clinical Counselor obtained prior to hire date or job transfer date.
    • Counselor obtained prior to hire date or job transfer date.
    • Social Worker credentialed from the Indiana Behavioral Health and Human Services Licensing Board obtained prior to hire date or job transfer date.
    • Social Worker specializing in Clinical Practice credentialed from the Indiana Behavioral Health and Human Services Licensing Board obtained within 12 Months (1 year) of hire date or job transfer date.
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
  • Case Manager credentialed from the American Case Management Association (ACMA) preferred.

Education:

  • Bachelor's degree in Social Work required.

Work Experience:

  • 3 years of experience preferred.
What additional requirements you'll need
  • Must have Social Worker license.
  • Masters degree in Social Work required.
  • Emergency Room experience
