Position Summary

The Resident Care Coordinator (RCC) is responsible for ensuring high-quality care and services for all residents in our assisted living community. This role plays a critical leadership function by overseeing daily care operations, supporting and developing care staff, and ensuring Charterโ€™s mission and values are integrated into all aspects of resident care.

The RCC works closely with leadership and reports directly to the Health and Wellness Director (HWD).

Key Responsibilities
  • Ensure quality, resident-centered care across the assisted living neighborhood

  • Supervise, schedule, evaluate, and support frontline care staff

  • Promote compliance with state regulations, company policies, and care standards

  • Assist with care planning, resident assessments, and ongoing monitoring

  • Serve as a clinical and operational resource to care staff

  • Foster a positive, engaged, and team-oriented work environment

  • Support family communication and resident satisfaction initiatives

  • Model Charterโ€™s mission and core values in daily practice

Qualifications
  • Active LPN license (required)

  • Assisted living experience strongly preferred

  • Leadership or care coordination experience preferred

  • Strong organizational, communication, and team-building skills
