Job description

SUMMARY:   Resident Care Coordinator (RCC) is a professional Care Team Member trained to provide and oversee personal care and related services to residents in the community according to documented, Individual Service Plan(s) ISP. Must have current certifications in Certified Nurse's Assistant, Certified Medication Aide, and/or Advanced Certified Medication Aide. The RCC also assist in the training of, supervision and motivation of CNAs and CMAs and other ancillary duties as described herein.  

ESSENTIAL DUTIES AND RESPONSIBILITIES:   

1.            The RCC is responsible ASSITING with the complete implementation of the ISP specific to each resident ensuring that person centered care is explained in a way that fosters consistent approach and results.

2.            The RCC ensures that any additional communication through the Policy and Procedure such as bath lists, laundry lists, daily task list, etc. reflect each resident's person-centered needs.

3.            The RCC reads shift to shift reports daily when on duty. The RCC will maintain these books as needed.

4.            The RCC monitors the task completion in electronic system at least weekly and monitors for changes in resident needs. Monitoring of changes includes verbally reviewing resident needs at least monthly to note any changes that may not have been documented. At least monthly a comparison of the ISP and actual care delivered should be performed, if changes are noted, these changes are reported to WD and ED in writing.

5.            May provide assistance with the following ADLs/Medication according to the resident's ISP:

a.            Personal hygiene (bathing, grooming, hair care, shaving, skin care)

b.            Change bed linens and make bed.

c.            Toileting and Incontinent Care

d.            Nutritional and fluid intake

e.            Dressing

f.             Ambulation and transfers

g.            Positioning

h.            Medication reminders

i.             Other duties as assigned within scope of practice

6.           Communicates with the Wellness Director about changes in the resident's status.

7.           Utilizes standard precautions and infection control procedures.

8.           Assists the resident in maintaining a safe and clean environment.

9.           The RCC updates resident care plans as requested.

10.         Maintaining resident records/charts, filing and archiving.

11.         Ensuring resident record/charts contain all state required documents, etc.

12.         Incident Reports (ensuring completion, responsible parties notified, submission, etc.)

13.         Follows written instructions and requests assistance and further instructions with new or unfamiliar situations.

14.         Salary position, on call for staffing and works weekend coverage.

15.         Performs any other duties as assigned.

LEADERSHIP:

1.            The RCC embraces the Jasmine Estates vision and core values and provides leadership that promotes these values.

2.            The RCC will actively solve day to day HR and care issues with the WD. The ED will provide direction/support as needed and all actions and issues shall be communicated to the ED (and WD as needed).

3.           Responsible for maintaining supplies/inventory for Assisted Living and Memory Care.

4.            The RCC will serve as the day-to-day coach and mentor to develop CNAs and CMAs ensure proper execution of their job duties.

5.            The RCC shall organize, schedule and monitor completion of monthly in-service training. This may include covering "the floor" for an CNA or CMA to allow that person to complete their required training program.

6.            The RCC will assist in delivering correspondence to support a corrective action with the approval of the ED or WD as needed.

7.            The RCC creates a monthly schedule for CNAs and CMAs on each shift, monitors overtime, documents absences, approves all PTO requests and track/trends when team members call in sick. Overtime and knowledge of residents should be key considerations when arranging for coverage. If coverage cannot be found, the RCC will work that shift. The RCC may not be allowed to work more than a double in any given workday and the ED will ensure that extra shifts are not excessive, providing support as needed.

8.           Exhibits a caring and respectful attitude toward the resident and the resident's property.

9.           Adheres to all community and company policies.

10.         The RCC attends monthly and participated in meetings.

11.         The RCC will be the primary contact for family and resident regarding care implementation needs

including requesting or ordering supplies per the resident/family preference and assist with all new move-in residents.

12.         Follows written instructions and requests assistance and further instructions with new or unfamiliar situations.

13.         Responsible for reviewing candidates and setting interview schedules; ensuring good communication with the leadership team.

HIRING:

1.           Responsible for checking with AED for new hire paperwork, completing reference checks, scanning files to HR, assembling, and maintaining team member files at the community.

2.            Responsible for coordinating and organizing new hire orientation with AED following the Jasmine Estates orientation program. Primary on the job trainer & team mentor for all new team members.

3.            Responsible for reviewing candidates and assist with interviewing with WD and ensuring good communication with the leadership team.

4.            Follows written instructions and requests assistance and further instructions with new or unfamiliar situations.

 

 

QUALIFICATIONSTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required. 

1.            High school diploma or a GED/ Be at least 18 years of age.

2.            Successfully completed an approved skills competency evaluation and written skills test per state        guidelines.

3.            Be able to read, write and comprehend English.

4.            Must have current certification of CNA, CMA and or ACMA

5.            Must have dependable means of transportation.

6.           Salary position, on call for staffing and works weekend coverage.

7.            Must pass criminal history check and must not be listed on the Employee Misconduct Registry/ NAR in Oklahoma only.

8.            Have a minimum of at least 2 years' experience in a caregiving role, preferably with the elderly.

9.            Agree to undergo supplemental training by Community personnel, as may be required by the Community.

10.         Have an interest in and a sympathetic attitude toward caring for the elderly.

11.         Has the ability to work within a multidisciplinary team setting.

12.         Employee may be subjected to hostile and emotionally upset residents, family members, personnel, visitors, etc.

13.         Regular attendance as prescribed by the operating hours of the community with exceptions for allowed absences per policy.

14.         Must be able to endure the resident's environment, which may be unsanitary and uncomfortable.

15.         Must be able to assist residents to roll in bed, sit up from a lying position, stand from a sitting position, transfer assist to and from w/c and provide stand-by assist during resident ambulation or other activities of daily living

Must possess a good sense of efficiency and in successfully completing all work assignments; must be knowledgeable and supportive of the Community's philosophy and objectives; must be flexible and open to change; must be knowledgeable of current regulatory standards; may be required to work extra hours, weekends and/or be on call during off hours.
