Highlights of the Memory Care Coordinator Position:
  • The Memory Care Coordinator oversees the overall implementation, delivery and coordination of the Connections Memory Care neighborhood resident services, including engagement and supports the MCD/HWD in non-clinical health services.
  • Coordinates engagement aspects of life for Memory Care residents including participating in the development of personalized plan or program for successful aging in community.
  • Ensures residents satisfaction and achievement of their realistic goals. Motivates residents to function at highest level of independence.

The salary range for this position is $17.00-$22.00 per hour.
