Job description

The Care Coordination Assistant Responsibilities Include:
  • Entering new referrals and updates all existing patient information in corporate IS systems including demographics, insurance, and benefit information.
  • Verifying insurance and benefits for all Medicare, Managed Care and Private Fee for Service Plans. Securing authorization for services provided by BayCare HomeCare from the payers.
  • Selecting the correct payer codes to provide accurate intake information.
  • Understanding medical criteria for securing all required information from the referral source.
  • Entering the appropriate financial information including benefits, deductibles, co-pays, coinsurance, out-of-pocket, another related costs into the EMR system.
  • Timely requesting and communicating of authorizations to payer and documentation in EMR.
  • The Care Coordination Assistant will work under the direction of the Care Transition Coordinator or RN Case Manager to establish and maintain a positive working relationship with patients, payors and other customers.

  • Location: Hybrid - Largo, FL
  • Status: PRN - As Needed
  • Schedule: 8:00 a.m. - 5:00 p.m., Monday - Friday
  • Weekend requirement: Occasional
  • On Call - No

Education:
  • Required High School Graduate or Equivalent GED
  • Preferred Technical Diploma - Medical Specialization

Experience:
  • Required 1 year Customer Service
  • Required 1 year Insurance Verification
  • Required 1-year Medical Related field
