Description
BridgeWater Assisted Living Community is currently seeking for a Resident Care Coordinator to join our team This position supports the Assisted Living Director with managing medications, audit med carts, provide basic care, assist on the floor, and handle resident data entry as needed.
  • The schedule for this position is Monday to Friday from 9:00 am to 5:00 pm

Responsibilities:
Maintains communication of all areas with Director of Assisted Living at all times.
  • Upon direction of the director, coordinates and conducts floor training of new staff, as well as scheduling of current staff.
  • Meets daily with the director to review resident care issues.
  • Communicate changes in the physical and/or emotional status of the resident and/or family to the Assisted Living Director
  • Serves as the staff and family resource when the director is unavailable or away from the building.
  • Provides skin and wound care.
  • Responsible for medication administration for the Assisted Living Department in compliance with Federal and State regulations and our company guidelines.
  • Performs routine medication cart audits to ensure quality control.
  • Ensures that employee tuberculosis testing (TB) is conducted in a timely manner.
  • Respects residents' confidentiality.
  • Accurately charts vital signs, refill chart forms, and maintain supplies, equipment and forms needed for the department
  • Keeps residents' records in proper order and sees that only authorized personnel have access.
  • Recognizes abnormal changes in body functioning and the importance of reporting such changes to Director of Assisted Living.
  • Provides assistance as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living.
  • Contributes to the resident's assessment and the plan of care.
  • Provides nursing rehabilitation /restorative nursing services.
  • Assist with master assignment sheets, call in books, ect.
  • Assist with staffing schedule, deliver all PTO forms to Director of Assisted Living for approval. Maintain master department PTO calendar in Director of Assisted Livings office.
  • Assist with shift coverage.
  • Performs other duties as assigned.

Requirements
  • RMA or CMA license/certification required.
  • Minimum of 1 year of related experience in Senior Living Community required.
  • Previous experience in similar role highly preferred.
  • Full Time Schedule.
  • A High School diploma or equivalent.
