Position Summary

Essential Duties and Responsibilities - Case Management Assistant

1. Maintaining accurate, up-to-date, and confidential client files and electronic medical/social records.

2. Preparing, processing, and submitting necessary paperwork (such as insurance authorizations, referrals, and government forms).

3. Organizing meetings, medical visits, and follow-up appointments for clients.

4. Keeping clients, their families, and care providers informed of ongoing progress and schedule changes

5. Helping clients access and navigate community resources, rehabilitation programs, and support services.

6. Assisting with pre- and post-discharge activities to ensure smooth transitions between facilities or into home care.

7. Following up with clients to ensure their basic needs and personalized assistance plans are being met

Competencies

  • Notifies team members and physicians on incomplete records upon discovery

  • Insures that Medicare Certification/Re-certification forms are created and signed by attending physicians in a timely, compliant manner. Demonstrates timely communication to CEO, or designee, all exceptions to compliance

  • Effectively utilizes resources to manage medical records oversight and all requests for record productions, including Smartsheet and Point Click Care

  • Ensures that requests for medical records copies are fulfilled timely, completely and accurately, in accordance with PureHealth and legal department, as indicated

Work Environment

This job operates in a post-acute setting. This role requires regular walking to various locations around the facility. This role also routinely comes into contact with patients who may have contagious illnesses. Team member is expected to adhere to facility’s no-smoking policy.

Physical Demands

While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift and/or move items over 20 pounds.

Position Type and Expected Hours of Work

Normal Hours: Monday - Friday 8am - 5pm

This is a position in a post-acute setting that is open 24 hours a day, 365 days a year. Weekend and holiday shifts will be required.

HIPAA

Team members are required to adhere to policies and procedures implementing HIPAA requirements for the privacy and security of protected health information. Team member are permitted to use and/or disclose only minimum amount of Protected Health Information necessary to complete assigned tasks. Reports all suspected violation of company’s HIPAA policies or procedures to facility CEO.
