Job Summary and Qualifications

The Case Management Assistant (CMA) will work under the direction of the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services, and will identify and report barriers to discharges. The CMA will provide administrative clerical support in Case Management functions.

· Coordinate with RN case manager and social worker for support needed. Communicate updates on progress throughout the day.

· Set up transportation via the Transfer Center and print discharge packets for post-acute care transfers.

· Use web-based referral system to manage post-acute care referrals.

· Prioritizes workload to ensure a timely contact and intervention with PAC providers.

· Monitors patients with post-acute care referrals and intervenes to expedite discharge.

· Communicate with patient and have the Patient Choice letter signed.

· Deliver the Import Medicare Message letter to the patient per facility policy.


    What qualifications you will need:

    · Associate Degree preferred

      · Medical knowledge and terminology is required 

      * Minimum of one year experience in a health care setting is also required, C.N.A license preferred

      * Possess strong organizational skills, comfortable in a fast paced environment, can multi-task along with excellent communication skills to communicate with patients, families and skilled nursing facilities

      * Possess the ability to work Monday - Friday, 8a - 4:30p
