Job description

As a Service Provider at Visions Case Management & Consulting, you will be strengthening the communities of New Mexico by enhancing the lives of those we serve and work with. Will also be a source of information, to foster a supportive environment, to deliver exceptional services, and to empower everyone you interact with.


Responsibilities:

You will have the following responsibilities:

  • Facilitate monthly contact with consumers and submit related documentation in a timely manner.
  • Drafting and submitting annual plans/budgets according to program timelines and revise as needed.
  • Maintain complete and current client case file.
  • Utilize and update internal tracking systems 2 x weekly and ensure information is current and complete. Specifically google sheets or spreadsheets in general
  • Participate in monthly staff meetings and either meet with your regional lead weekly or program manager monthly for supervision and training.
  • Respond to internal and external communications within 24 hours.



Required Qualifications:

  • Four Year Degree in Nursing, Psychology, Social Work and 1 year of related experience
  • Any Four Year Degree and 6 years of related experience
  • Travel entire State of New Mexico
