What is a Community Health Representative?

A Community Health Representative (CHR) is a trained public health worker who serves as a vital link between healthcare providers and the community, especially in underserved or rural areas. CHRs help educate individuals and families about health resources, provide basic health services, and assist with navigating the healthcare system. They often work within specific communities, such as Native American tribes, to address local health needs and promote wellness. Community Health Representatives (CHRs) work closely with healthcare providers, social workers, and local organizations to bridge gaps in care and improve health outcomes within their communities.
