PrimaryOne Health is currently seeking a Behavioral Health Outpatient Clinician II for our Columbus health centers.
Essential Duties and Responsibilities:
  • Performs diagnostic and psychosocial needs assessments on adult, pediatric, obstetric, gynecologic, and geriatric patients.
  • Provides risk assessment and interventions on patients connected with behavioral health services.
  • Provides counseling services when deemed appropriate.
  • Works with the healthcare team to enhance patient-centered care received by the patient.
  • Provides education to the team regarding the patient's behavioral health concerns of the effect of the patient's social situation on their health.
  • Assists with linking patients to needed social resources, including advocating on their behalf with external agencies and institutions.
  • Works collaboratively with MSW internship training program.
  • May be responsible for attending various community meetings and/or events as a PrimaryOne Health representative as assigned.
  • Occasional travel to various PrimaryOne Health center sites required.
  • Other duties or special projects as assigned.

Successful candidates must possess a master's degree in social work or counseling and a license to practice in the State of Ohio (LISW, LISW-S, LPCC, LPCC-S, LMFT, IMFT, IMFT-S). Two years' experience in Behavioral Health (mental health and substance use); Integrated care, crisis intervention, or medical setting experience preferred (can include internship).
