A Community Health Representative (CHR) is a trained public health worker who serves as a vital link between healthcare providers and the community, especially in underserved or rural areas. CHRs help educate individuals and families about health resources, provide basic health services, and assist with navigating the healthcare system. They often work within specific communities, such as Native American tribes, to address local health needs and promote wellness. Their work helps improve access to care, reduce health disparities, and enhance overall community well-being.

How does a community health representative typically collaborate with healthcare providers and local organizations?

Community Health Representatives (CHRs) work closely with healthcare providers, social workers, and local organizations to bridge gaps in care and improve health outcomes within their communities. They often help coordinate care by communicating patient needs, assisting with appointment scheduling, and facilitating access to resources like transportation or health education. CHRs also participate in team meetings and case reviews to ensure patients receive comprehensive, culturally appropriate support. This collaborative approach is central to their role and helps build trust between the community and healthcare systems.
